If you did not apply using the online grant portal, please use the downloadable report templates below. If you are not sure which type of grant your organization has received, please refer to your grant correspondence or email grants [at] pghfdn.org.
- To submit a report for an unrestricted grant from The Pittsburgh Foundation, please choose from the interim or final report template.
- To submit a report for a medical research grant from The Pittsburgh Foundation, please use the medical research template.
- If you have received a donor-advised or designated grant and have questions about required follow-up reports, please inquire via email to grants [at] pghfdn.org (include the grant number and fund name in the email).
