The Forbes Funds invites interested organizations to attend this free session which will bring you up-to-date on three key topics:

  • Affordable Care Compliance Requirements. Discussion of the compliance requirements slated to take effect in January 2015 for organizations that employ more than 50 people, and how to be sure you have the necessary systems in place for reporting and healthcare coverage. 
  • I-Count Project Team. We will report on activities of both the local I-Count project team and the Commonwealth of Pennsylvania in pursuing funding for a State Health Innovation Model grant in which I-Count hopes to be a participant.
  • Request For Proposal. The Forbes Funds will distribute a formal RFP at this meeting, inviting applications to participate as partners or affiliates in this initiative.

The I-Count model emerged from the work of 25 agencies drawn from primary care, human services and behavioral health over the summer of 2013. It is designed to be a patient-centered model of care that improves healthcare outcomes, utilizes the expertise of existing community providers, reduces inappropriate use of emergency rooms, and readmissions for people with complex needs. The model will be administered in Pittsburgh by an accountable care organization. I-Count expects to serve 10,000 adults, who:

  • Are insured by Medical Assistance, Medicare Advantage (including special needs plans) or uninsured,
  • Frequently use hospital emergency departments for basic health care and/or experience frequent and repeated hospital admissions for conditions that could be addressed at a lesser treatment level,
  • Have difficulty with following recommended treatment plans due to lack of understanding, social support, or other issues,
  • Have low incomes.

At this time The Forbes Funds is seeking agency partners and affiliates to participate as service providers in one or more of I-Count’s three proposed demonstration sectors in the city of Pittsburgh, namely:

Sector 1:  East Liberty, Larimer, Lincoln/Lemington/Belmar, and Homewood
Sector 2:  Oakland, Squirrel Hill, Greenfield, Hazelwood, and Glen Hazel
Sector 3:  North Shore, East and West Allegheny, Central Northside, Fineview, California/Kirkbride,  Marshall/ Shadeland, Northview Heights, Perry North and South, and Spring Hill.
